Introduction
MyDocLine, LLC d/b/a MyMDLine (“MyDocLine”, “us”, “we”, “our”) is strongly committed to
safeguarding the privacy of our users and protecting that privacy through our compliance with
this policy. This Privacy Policy describes the type of information we may collect from you or
that you may provide when you use the services through the MyDocLine site (the “Service”) and
the use and disclosure of that information for those who decide to use the Service. Please read
this Privacy Policy carefully before using the Services with MyDocLine.
Note: MyDocLine is not a medical group. Any telehealth consults obtained through our
Services are provided by independent medical practitioners (a “Provider”). Each Provider
is bound by contractual obligations to keep personal information confidential and use it
only for the purposes for which we disclose it.
If you choose to use our Service, then you agree to the collection and use of information in
relation to this policy. The Personal Information that we collect is used for providing and
improving the Service. We will not use or share your information with anyone except as
described in this Privacy Policy.
This Privacy Policy is incorporated into and subject to our Terms of Use (“Terms of Use”). Each
time that you access or use the Services, you signify that you have read, understand and agree to
be bound by the Terms of Use and this Privacy Policy. If you do not agree to the Terms of Use
and this Privacy Policy, you must discontinue using our Services.

HIPAA. In addition, to the extent applicable to the Services provided to you, the
healthcare providers from the MyDocLine that you interact with when using the Services are
required to comply with the Health Insurance Portability and Accountability Act (“HIPAA”),
including with respect to patient privacy. In addition, our third party application service
provider, VSee, which we use to support our business and which is bound by contractual
obligations to keep personal information confidential and use it only for the purposes for which
we disclose it to VSee, also is HIPAA compliant..

Third Party Sites. Our site and application may provide you with hyperlinks to other
sites not owned or controlled by us and we are not responsible for the privacy practices of any
site that is not owned or controlled by us. We do not share personal information with those other
sites, unless specifically authorized by you. We aim to work with trusted partners and
organizations which are also bound by state and federal laws governing privacy and security;
however, we encourage users to be aware when you leave our site or mobile application and to
review any privacy policy of any third party site.
Important Definitions. When we use the term “Personal Information” in this Privacy Policy, we
mean information about you that is personally identifiable to you, such as your contact
information (e.g., name, address, email address, or telephone number), personally identifiable
health or medical information (“Health Information”), and any other non-public information that
is associated with such information (collectively, “Personal Information”). When we use the term
“De-Identified Information”, we mean information about you but that does not identify you
(“De-Identified Information”). Payment information (“Payment Information”) is information
you provide that corresponds to a financial account that we use to process payments from you,
such as your credit card account.

Types of Information We Collect
The only information we obtain about our users is information required to properly use the
services provided by MyDocLine, as well as information required to process credit card charges
for transactions.
Information you provide when you register for or modify your MyDocLine account and
use our Services by which you may be personally identified, which may include your
name, age, gender, email address, password, audio, images and videos of you, and other
such information.
Health Information that you provide us, which may include information or records
relating to your medical or health history, health status and laboratory testing results,
diagnostic images, and other health related information.
Health Information about you prepared by the health care provider(s) who provide the
Services through the Site such as medical records, treatment and examination notes, and
other health related information.
Payment Information that you provide us. Although you submit your Payment
Information in order to use the Services, MyDocLine does not store or process your
payment information. Your payment information is submitted directly to and stored by
our payment’s processor, [Stripe]. MyDocLine chose [Stripe] as its payments processor
in part because of its ability to secure your payment information in compliance with the
Payment Card Industry Data Security Standard. You should familiarize yourself with
Stripe’s privacy and security policies if you have concerns about your payment
information.
Information about the computer or mobile device you are using, such as what Internet
browser you use, the kind of computer or mobile device you use, and other information
about how you use the Site; and
Any other information you input into the site or related services.
In the event you disclose any of your Personal Information in a public forum on our site, the
information in that disclosure will be treated as public, we will not take any steps to protect,
secure, or delete that disclosure, and you should not have any expectation of privacy or
confidentiality with regards to that disclosure

How We Collect Your Information
We may collect all of the information you provide to us through the activities described
above.
When you access the Services from a computer or other device, we may collect
anonymous information from that device, such as your Internet protocol address, browser
type, connection speed and access times.
We do not collect information through technology, such as cookies when you visit our
site or open emails from us. However, when you access our Service through our
application service provider, VSee, they may collect information through Cookies and
you should be familiar with their privacy policy when you access that application.

How We Use Your Information
We may use your Personal Information for the following purposes (subject to applicable legal
restrictions):
Provide you with the services you requested;
Improve our products, services and operations;
To create De-identified Information such as aggregate statistics relating to the use of the
Service;
To notify you when site updates are available;
To market and promote our site and the Services offered to you through the site;
To fulfill any other purpose for which you provide us Personal Information;
Detect, investigate and prevent activities that may violate our policies or be illegal;
Review whether it is responsive to a subpoena or other legal request we have received;
and
For any other purpose for which you give us authorization.
Some information MyDocLine collects constitutes protected health information (“PHI”) under
the U.S. Health Insurance Portability and Accountability Act (“HIPAA”). . MyDocLine will use
PHI only for the purpose for which it was collected and we only collect the information we need
to fully perform our services and to respond to you or your Provider or your insurance company.
We may use your PHI to contact you, to provide requested services, to provide information to
your Providers, to obtain payment for our services, to respond to your inquiries and requests, and
to respond to inquiries and requests from your Providers. PHI will not be used for any other
purpose, including marketing, without your consent. Any PHI used by MyDocLine for purposes
of the Services will comply with HIPAA.
We only use your Payment Information to facilitate your subscription payment and receipt of
payments from Customers through third party companies.
We may de-identify or aggregate your Personal Information so that it no longer reasonably
identifies you. In this case, we may use this De-Identified Information without restriction and for
any purpose, including to improve our Services.

When We Share Your Information
We may share your Personal Information with other companies in limited circumstances. The
instances of when we may share your personal information with others include:
When you direct us to share your information with third-party sites or platforms or with
your consent.
To contractors, insurance companies, service providers and other third parties we use to
support our business and who are bound by contractual obligations to keep personal
information confidential and use it only for the purposes for which we disclose it to them.
As required by law, which can include providing information as required by law,
regulation, subpoena, court order, legal process or government request.
When we believe in good faith that disclosure is necessary to protect your safety or the
safety of others, to protect our rights, to investigate fraud, or to respond to a government
request.
To a buyer or other successor in the event of a merger, divestiture, restructuring,
reorganization, dissolution or other sale or transfer of some or all of MyDocLine‘s assets,
whether as a going concern or as part of bankruptcy, liquidation or similar proceeding, in
which Personal Information maintained by the Services is among the assets transferred.
Please note that once we share your information with another company, the other company’s
privacy practices will govern their use of the information.

Do Not Track Signals
Some web browsers permit you to broadcast a signal to websites and online services indicating a
preference that they “do not track” your online activities. At this time, we do not honor such
signals and our third-party websites or other online services may collect information about your
online activities. The information gained from these signals is used as outlined in Section 6.

Security of Your Information
To prevent unauthorized access, maintain data accuracy, and ensure the appropriate use of
information, MyDocLine has put in place appropriate physical, electronic, and managerial
procedures to protect the information we collect. Please understand, however, that while we try
our best to safeguard your personally identifiable information once we receive it, no transmission
of data over the Internet or any social media or other public network can be guaranteed to be
100% secure.
You need to help protect the privacy of your own information, including maintaining the
confidentiality of any account information or access credentials. You must take precautions to
protect the security of any personally identifiable information that you may transmit over any
public or untrusted network by using encryption and other techniques to prevent unauthorized
persons from intercepting or receiving any of your personally identifiable information. You are
responsible for the security of your information when using unencrypted, open access, public or
otherwise unsecured networks.
In the event you disclose any of your personal information in a public forum, the information in
that disclosure will be treated as public, we will not take any steps to protect, secure, or delete
that disclosure, and you should not have any expectation of privacy or confidentiality with
regards to that disclosure.
10.
How Long Your Information is Stored
MyDocLine will continue to store and process your personal information until the earlier of (i)
you request that we delete your personal information in accordance with the procedures set forth
in this privacy policy, or (ii) the end of any requirement period provided under the rules of
HIPPA.
11.
How You Control Your Information, Modifications, Opting-Out and Cancellations.
You may modify, change, or correct your information in the same manner in which you
originally provided it to us.
You may Opt-Out of any of the Services or the use of your Personal Information in any way by
contacting us at info@mydocline.com.
You can close your online account by emailing us info@mydocline.com. If you close your
account, we will no longer use your online account information or share it with third parties. We
may, however, retain a copy of the information for archival purposes and in accordance with any
HIPPA requirements.
With respect to any PHI MyDocLine may obtain, you have certain rights under HIPAA to access
your data, to restrict use and disclosure of it, to request communication methods, to request
corrections to your data, to receive an accounting of disclosures and to receive notice of any
breach.

Report Violations
You should report any security violations to us by sending an email to _info@mydocline.com.
13.
A Special Note About Children
MyDocLine is directed to a general audience and not directed to those under the age of 18. We
ask that minors not submit any personal information. MyDocLine does not knowingly collect
personal information from individuals under 18 years of age and if we obtain actual knowledge
that a user is under 18 years of age, we will take steps to remove that user’s personal information
from our systems.
If you believe that we have inadvertently collected personal information from a child under 18,
please contact us at info@mydocline.com so we can take appropriate steps to delete the child’s
personal information.
14.
California Privacy Rights
At this time, MyDocLine has determined that it is not subject to the California Consumer
Privacy Act of 2018 (CCPA) pursuant to Assembly Bill 713 amending Section 1798.146 of
the California Civil Code relating to consumer privacy.
15.
Special Policies For EU Residents
In accordance with its obligations under the EU General Data Protection Regulation,
MyDocLine sets forth the following additional notices and policies with respect to the residents
of the EU who use its services.
Purposes For Collecting Personal Information
MyDocLine only collects and uses your personal information only where it has a legal
basis to do so, including:
MyDocLine needs it to provide you the Online Services and MyDocLine’s other
services, including to provide customer support;
MyDocLine needs it to satisfy a legitimate interest (which is not overridden by
your data protection interests), such as for research and development, to market and
promote MyDocLine services and to protect MyDocLine ‘s legal rights and interests;
MyDocLine has obtained your specific consent to do so for a specific purpose; or
MyDocLine needs to process your personal information to comply with a legal
obligation.
Your Rights To Control Your Information
To exercise any of your rights set forth in this section, please contact us at
info@mydocline.com. Subject to our authentication of your identity, we will act on your
request promptly.
Right to Withdraw Consent and Stop Our Use of Your Personal Information: If you
have consented for MyDocLine to use your personal information for a specific purpose or
purposes, you may contact us to instruct us that you withdraw your consent that we use
your personal information for that purpose or those purposes. Note however, this will not
affect any processing or use of your Personal Information that took place before you
withdraw your consent.
Right to Access and Port Your Personal Information: You may contact us to receive
a copy of your Personal Information, which you may then port elsewhere for your
purposes.
Right to Correct or Change Your Personal Information: If your Personal Information
has changed or you discover our records of your Personal Information is otherwise
incorrect, you may contact us to change your Personal Information accordingly.
Right to Be Forgotten: If you do not wish for MyDocLine to have your Personal
Information anymore, you may contact us to instruct us to delete your Personal
Information from our records. We will delete your Personal Information from our
database. Note, however, if you have communicated to us through email or other
channels, we may incidentally have portions of your Personal Information within our
internal archives of such communications, however, we will not use such Personal
Information for any purpose inconsistent with your right to be forgotten or the
record-keeping purposes of the archive. We may also preserve your Personal
Information if we are required to do so by law, however, we will not use such Personal
Information for any purpose inconsistent with your right to be forgotten or such legal
obligation.
Right to Review Decision-Making: If you question why you have received a
communication or other material from MyDocLine, you may contact us to learn what
about your personal information led us to send you the communication or other material.
16.
Changes to Our Privacy Policy
From time to time, MyDocLine may update its Policy. MyDocLine will post the new Policy with
a new Effective Date to notify you of these changes. MyDocLine will provide notice to you, and,
if MyDocLine is required by applicable law, MyDocLine will obtain your consent. If
MyDocLine has not obtained your consent, you are deemed to consent to our then-current Policy
by accessing the Services after the Effective Date. If you are concerned about how your
information is used, you should check back to this Policy frequently for updates. Unless you are
an EU Resident for whom different methods of consent apply (as discussed in the Section 14
above), by accessing the Services after the Effective Date, you are deemed to consent to our
then-current Policy.

Choice of Law
The laws of the State of Illinois shall govern any dispute, including those disputes arising from
MyDocLine ‘s use of Personal Information or otherwise relating to privacy, as specified in the
Terms
